OPPORTUNITIES FOR EMPLOYERS IN DIRECT CONTRACTING AND CANCER CARE MANAGEMENT
Self-insured employers are struggling. They seek to purchase the correct care at a fair price in a rapidly evolving treatment marketplace. Even trusted intermediaries have a hard time keeping up with the pace of change. Meanwhile, cancer has become the major driver of high-cost claims, and the incidence of the condition impacting the lives of plan members is growing. Direct contracting with providers and focused intermediaries can help them keep up while reducing costs can be a solution. Reporting that helps them understand cancer care delivery in the marketplace and its impact on disability, as well as care costs, can provide a sense of greater managerial understanding than is available today.

HOW ONCOLOGY BECAME THE “UNPROTECTED” CLASS AND WILL IT REMAIN?
Oncology has long held privileged status as one of Medicare Part D’s six protected classes, but that protection is now under quiet siege. With 9 key cancer agents already affected by negotiation and more to come, CMS is forcing price erosion and bending formulary dynamics in key tumors. Part D benefit redesign and PBM reform are fundamentally altering payer incentives. CMMI proposed MFN models explicitly target protected classes. This panel will debate a hard question: Is oncology still a protected class in a meaningful sense, and what does the next wave of access barriers look like for patients, prescribers, and manufacturers?

USING TECH TO OPTIMIZE CANCER THERAPEUTICS: REDUCING TOXICITY, ENSURING ADHERENCE, AND MAXIMIZING BENEFIT
Although our therapeutic armamentarium has dramatically improved, we continue to be faced with how best to optimize the treatments we have available. Bispecific, CAR-T, and complex regimens are moving into community oncology faster than the infrastructure to support them. Monitoring adherence to our growing list of oral cancer drugs remains challenging. Even well-established infused therapeutic agents can be prematurely discontinued if toxicity isn’t recognized and managed. This panel examines whether tech-enabled workflows can solve these challenges, allowing our patients to derive the greatest possible benefits from these life-saving treatments.

SAFE HARBOR DRUG CONTRACTING: IS THIS THE RIGHT CONTRACT INNOVATION TO REPLACE DISCOUNTS, REBATES, AND PROVIDE TRANSPARENCY?
On Nov. 20th, 2020, the Trump Administration revived the Rebate Safe Harbor Rule in effort to reform drug pricing. Safe Harbors offer an Innovative contacting solution in which economic risk can be shared by pharmaceutical manufacturers and the economics for contracting risk does not collapse Net Cost Recovery. Unfortunately, marketplace adoption as been almost no existent. Safe harbor regulations (42 C.F.R. § 1001.952) protect specific pharmaceutical drug contracts and warranty arrangements from violating the Federal Anti-Kickback Statute (AKS). These regulations ensure that legitimate, value-based contracts—such as manufacturer rebates, price reductions, and outcome-based warranties—are not misconstrued as illegal remuneration, provided they strictly meet regulatory. Discussion will focus on Key Safe Harbor Protections for drug contacts such as Warranty Safe Harbor (42 C.F.R. 1001.952(g)): Protects arrangements where a manufacturer reimburses a payer (or patient) if a drug fails to work as intended, often used for high-cost cell or gene therapies such as Discount Safe Harbor (42 C.F.R. 1001.952(h)) which covers reductions in price, including rebates for prescription drugs, provided they are properly disclosed and reported and Outcome-Based Agreements: Recent updates to the Personal Services and Management Contracts safe harbor now cover value-based arrangements where compensation is tied to clinical outcomes, rather than just volume. This panel of legal experts will discuss the possibilities that exist and providers and Pharma look to find models of contacting that produce the transparency the market is seeking.

STANDARDIZING SECOND OPINIONS: WHY DON’T WE GET FIRST OPINIONS RIGHT
Up to half of cancer patients seek a second opinion, and 20–58% of those reviews change the diagnosis or treatment plan, evidence that the first opinion too often misses the mark. The cost is both clinical and financial: avoidable variation, low-value care, and outcomes that diverge sharply by geography, with rural patients bearing the heaviest burden. This panel examines how expert subspecialist input paired with systematic case audit closes that variation, reduces low-value care, and improves outcomes, and what it means for value-based care. The panel will discuss where AI decision-support and pathway tools raise the floor on second opinion quality and will question whether a human expert check remains essential rather than obsolete. Finally, the panel will explore how technology is redrawing the map of oncology expertise so that life-saving knowledge reaches patients faster.

CAN INDUSTRY LOOK TO MOVE FROM GROSS–TO–NET REBATE DISCOUNTS TO OUTCOME–BASED DRUG CONTRACTING AGREEMENTS
Physician and Payers have tried to do Value Based Agreements before while stunningly the drug which represent 60-70% of the cancer care cost have not been an active participant in these contracts. The old school historical legacy gross to net discount rebate driven contracting has fallen out of favor by both the government healthcare policy and private healthcare markets. More recently the promise of Value Based Contacting has been nothing more than semantics and has failed to truly drive meaningful changes by aligning economic exchange of value between contracting parties. The failure to achieve meaningful Value Based Contact was anchored in the fact manufacturers and payers could not get the real-world data to compute the math required to operate VB agreements. Today and tomorrow as healthcare markets move to real world data interoperability and transparency the game will change. This panel will discuss innovative Outcome Based Agreements as a meaningful way to contract for value and ensure compliance with the new policy and marketplace demand for real world performance and price transparency is achieved without punishing stakeholders.

BENEFIT CONSULTANTS: ARE THEY LEADING OR MISLEADING THE MARKETPLACE?
Healthcare benefits consultants heavily influence how trillions of dollars are spent on employee health plans, and their guidance can result in massively beneficial or highly detrimental corporate outcomes. While a good consultant acts as a strategic fiduciary to optimize costs and care, a bad or misaligned partner can lock employers into expensive, conflicted structures that drain company resources. Benefit consulting firms have a mixed impact on cancer care, doing well in expanding access to specialized programs, but falling short by sometimes prioritizing restrictive cost controls. This panel expose what is working and what is wrong with a forward look as to what their leadership looks like in the future to building comprehensive employee cancer benefits -screening, treatment access, COE programs, and financial protection.

ARE NCCN AND PATHWAY ORGANIZATIONS THE REAL ACCESS DECISION MAKERS IN THE FUTURE?
Most payer institutions employ an oncology pathway that directs prescribers to preferred treatment options for their cancer patients. Integrated Delivery Networks, and provider health systems may use their own customized pathway while many others rely on NCCN, as a peer-reviewed evidence–based compendia to support drug selection. The evidence used in these pathways serves as the foundation for supporting policy and access. However, the environment is changing and NCCN recently clarified their position that same line of therapy step therapies are not clinically recommended. The session explores the influence of pathway organizations on access and how this will continue to change / adapt as scrutiny on utilization management programs persist

ALGORITHM TO OUTCOME: HOW AI IS RESHAPING THE PATHWAY-TO-VALUE-PIPELINE IN ONCOLOGY
The AI Landscape today from pathway-to-value pipeline, point-of-care decision support, utilization management, outcomes tracking and VBC contract operationalization by separating real-world impact from aspiration. Panel explores Dynamic Pathways vs. Static Guidelines, updating treatment recommendations in near real time; pathway committees on clinical breakthroughs or a governance crisis, and who is accountable when a dynamic recommendation diverges from published guideline? AI as the Missing Infrastructure for VBC: Outcomes-based contracts that have consistently collapsed under data complexity and attribution disputes. Will AI infrastructure layers make value-based contracting scalable? Can AI bridge the Data Divide so consolidated networks can demonstrate outcomes and negotiate stronger VBC contracts as smaller community practices largely cannot. FROM POINT SOLUTIONS TO PRACTICE INFRASTRUCTURE: WHERE AI IS GOING
The first wave of AI in oncology has produced a growing number of point solutions: tools for documentation, trial matching, patient engagement, prior authorization, imaging, coding, and clinical decision support. But oncology care is not experienced as a set of disconnected tasks. It is a complex, longitudinal workflow involving physicians, patients, nurses, pharmacists, payers, life sciences companies, and practice operators.
This panel will examine whether AI can become a connective layer across that ecosystem. The discussion will focus on how AI can support the full arc of oncology care, from pre-visit planning and treatment decisions to care coordination, reimbursement, toxicity monitoring, and real-world evidence generation.

THE IRA IS ALREADY REPRICING ONCOLOGY SO WHY IS THE INDUSTRY STILL OPERATING LIKE IT ISN’T? ARE DRUGS UNDER THE IRA ZOMBIES
The Inflation Reduction Act is no longer a future policy discussion. It is actively reshaping oncology economics today. Medicare drug price negotiations, inflation rebates, and shifting Part B and Part D incentives are already influencing manufacturer strategy, payer forecasting, and provider economics. Yet many stakeholders continue to make decisions as though the pre-IRA environment still exists.
Attendees will gain an understanding of how IRA-driven reimbursement changes will affect innovation, access, practice economics, and value-based payment models.

RADIOPHARMACEUTICALS: CLOSING THE SCIENCE-TO-PRACTICE GAP BEFORE ACCESS FALLS BEHIND
Radiopharmaceuticals are rapidly reshaping cancer care, but the central question is whether the healthcare system can deliver these therapies to eligible patients at scale. This panel will examine the widening gap between scientific promise and real-world implementation, with particular focus on the operational, reimbursement, infrastructure, and site-of-care barriers that could limit access if stakeholders do not act proactively. Science is advancing faster than operations, reimbursement, and care delivery can keep pace. CAR-T demonstrated that transformative clinical outcomes alone are not sufficient to guarantee broad adoption; operational complexity, financial uncertainty, and limited delivery infrastructure prevented many eligible patients from receiving treatment. Theragnostic and RLT risk repeating that trajectory unless stakeholders align earlier around implementation. Further, while most eligible patients are treated in community settings, infrastructure remains concentrated in academic medical centers. This site-of-care mismatch is one of the most important barriers to solve as radiopharmaceutical use expands.

US VS. CHINA, PRESERVING OUR NATIONAL TREASURE: CLINICAL RESEARCH
China has pursued global leadership in biotechnology for many years and has made tremendous headway in the past decade. Although the US has historically retained a leg up in drug discovery, it’s losing ground to China in manufacturing and clinical trials. In 2021, China’s global share of Phase 1 trials surpassed the United States’ share for the first time. In 2024, China surpassed the United States in the total number of clinical trials registered, with 39 percent of global trials. China’s goal is nothing less than manufacturing dominance, and much of the world’s advanced-stage manufacturing supply-chain infrastructure is now China based. This panel will explore what can be done to protect America clinical research and barriers we need to address and what solutions we have the power to implement.

IF PRIOR AUTHORIZATION IS DYING, WHAT REPLACES IT?
Nearly every stakeholder agrees that traditional utilization management creates administrative burden, delays care, and generates provider frustration. At the same time, oncology costs continue to rise and payers remain accountable for managing spend. As CMS, states, and industry groups push for prior authorization reform, the question is no longer whether utilization management should change, it is what should replace it. The session will explore the next generation of oncology cost management: pathways, prospective clinical guidance, navigation-driven intervention models, AI-enabled decision support, and risk-bearing care management organizations. The discussion should move beyond criticism of prior authorization toward practical alternatives.

OUTCOMES-BASED CONTRACTS IN ONCOLOGY: WHO OWNS THE OUTCOME?
We’ve been talking about outcomes-based contracts in oncology for years. In reality, most haven’t scaled and some haven’t lasted. Payers want real accountability on high-cost therapies, especially when benefit is still evolving at launch. Manufacturers want access, but push back on endpoints they don’t fully control. Providers are being asked to track and report outcomes without getting paid to do it. This session gets into what’s actually worked and what hasn’t. Where have these contracts held up? Where have they quietly fallen apart? What endpoints are people actually willing to stand behind? And is real-world data usable enough to support this and how can we evolve the model for a win-win-win position? Outcomes-based agreements (OBAs) are contracts where payment or compensation is tied to the successful achievement of specific, predefined results rather than for time spent or services rendered. This model shifts financial risk from the buyer to the provider, ensuring accountability and aligning incentives.

COLLAPSING THE WALL BETWEEN RESEARCH AND ROUTINE CARE
Why is “Research” still treated as a separate silo from “Care”? In the current oncology landscape, the artificial barrier between controlled clinical environments and the messy reality of a patient’s home remains a primary bottleneck for innovation. When every patient interaction is not captured as a structured data point, the ecosystem loses the invaluable “Real-World” experience that defines the true efficacy of a therapy. This session addresses the “Phase ¾ Cliff”—the phenomenon where drugs that perform exceptionally in pristine, small cohorts lose significant value due to unmanaged toxicities and poor adherence when introduced to the general population. The transition from a controlled trial to routine care is where value is either realized or lost. This session explores a radical convergence: embedding research-grade longitudinal data collection directly into the daily clinical workflow. By creating a “Living Laboratory,” providers can execute pragmatic trials at scale, identifying side effects in large cohorts—particularly those on complex oral therapies—far faster than traditional retrospective registries. The goal is to move from “studying” patients to creating a continuous feedback loop that informs care and regulatory evidence in real-time.

Oncology’s supply channel is being asked to deliver far more than a price. THE IRREPLACEABLE PARTNERSHIP: HUMAN AND AI IN HEALTHCARE
Practical approaches to integrating emotional intelligence, creativity, contextual judgment, institutional knowledge, and physical agency to unlock AI’s full potential.
Healthcare depends on capabilities no technology can fully replicate: emotional intelligence, creativity, contextual judgment, institutional knowledge, and physical agency. These qualities are not peripheral to healthcare economics; they shape how organizations solve problems, design workflows, and manage patient access, provider operations, and revenue cycle performance. As AI advances, the central challenge is not simply where to deploy it, but how to build the foundation for effective partnership between humans and AI.
This panel examines the practical steps organizations are taking to build a foundational AI strategy: how to identify the right starting point, structure early human-AI partnership models, strengthen solution development with AI rather than simply delegate work to it, assess and strengthen the security posture required to support AI adoption, and embed healthcare data privacy and security from the start.

FINANCING CAR-T & GENE THERAPY: INPATIENT VERSUS OUTPATIENT REALITIES
CAR-T is the hottest topic in cancer, but no one is covering their actual costs on this innovative therapy because it is very labor intensive for the physicians and institutions alike. The academic institutions have no idea what the finances of a CAR-T patient means for their bottom line and the payers are terrified of per patient costs that run from $2-5 million. Providers and CAR-T manufacturers are interested in reaching more patients closer to home. More than a year since REM requirements were removed for CAR-T and nearly one year since FACT Community Standards were released, have we seen meaningful movement? This panel will surface the market realities and offer up scalable solutions so healthcare markets can address patient demand responsibility.

HOW WILL PHARMACEUTICALS BE EVALUATED ON VALUE IN THE TRANSPARENCY WORLD
As price transparency requirements strip away the list price fiction (hospital price transparency, state drug pricing transparency laws requiring WAC increase reporting, PBM rebate disclosure), the negotiating leverage that used to hide behind opaque rebates and net pricing starts to erode. Payers, employers, and increasingly patients can see the gap between list and net, which shifts the burden of justification from “what does it cost” to “what does it do.” The open question is whether manufacturers get ahead of this by generating their own real-world evidence and outcomes data proactively, or whether third-party assessors (ICER, payers’ own P&T committees, eventually CMS under IRA-adjacent authorities) fill that vacuum and set the terms unilaterally. Once the pricing becomes visible, the value story has to hold up on its own; there’s no rebate structure left to paper over a weak evidence base.

340B PRE-ADJUDICATION: TRANSFORMING DRUG PRICING THROUGH PROSPECTIVE CLAIM MANAGEMENT: PAYER AND PROVIDER PERSPECTIVE
AS the 340B landscape evolves toward rebate-based administration, covered entities , manufacturers and payers face mounting challenges involving upfront drug acquisition costs, duplicate discounts, fragmented claim data, dispute resolution and retrospective reconciliation.
This session explores 340B claim pre-adjudication as a viable alternative to traditional administration models—enabling earlier eligibility verification, persistent claim identification, dispute resolution and coordinated financial transactions across the pharmaceutical supply chain.
Key discussion points
Rethinking the 340B financial model: Examine how verified claims and coordinated purchasing could reduce or eliminate upfront Wholesale Acquisition Cost (WAC) exposure for covered entities.
Establishing a single source of truth: Explore how point-of-sale verification and persistent 340B claim identifiers can strengthen transparency, prevent duplicate discounts, and improve manufacturer visibility.
Connecting claims to purchasing and payment: Evaluate opportunities to integrate eligibility determination, wholesaler transactions, rebate administration, reconciliation, and dispute resolution.
Addressing stakeholder concerns: Examine providers concerns on the potential for payer-based discriminatory pricing, data privacy risks, and the safeguards necessary to protect covered entities and patients.
Moving from concept to implementation: Demonstrate how existing technology can support prospective, transaction-level administration and identify the operational and policy barriers to broader adoption.
Key takeaway: An examination of whether prospective 340B claim management can reconcile covered entities’ financial needs with manufacturers’ requirements for transaction integrity, compliance, and accountability while also taking into considerations PBMs/payer’s needs.

PREPARE, REACT, OR BOTH? MARKET IMPLICATIONS AT THE INTERSECTION OF PATENT EXPIRY AND PRICE REFORM
As the PD-1 class faces a historic inflection point — with loss of exclusivity arriving in 2028 and Most Favored Nation pricing following close behind — manufacturers, payers, and providers are navigating a compounding set of pressures unlike anything oncology has seen before. This discussion will explore how key stakeholders are preparing, what scenarios are most likely to unfold, and what the downstream consequences mean for the broader market: LOE & MFN: By the numbers, when and how will LOE and MFP be introduced, and how will that compare to current state? Implications: What potential future scenarios exist, and how are manufacturers, payers, and providers preparing today? Near- and Long-Term: What is the likely impact / second-order effects on innovation, and how can stakeholders collaborate to support sustainability?

PBM, GPO, AND WHOLESALER CONTRACTING: HAS GOVERNMENT LITIGATION AND SETTLEMENTS CHANGED THE MODEL?
Government litigation and settlements involving PBMs, GPOs, and wholesalers, scrutinize PBM vertical integration. Simultaneously, DOJ settlements over generic drug pricing and distribution practices, have put long-standing contracting norms under a spotlight they had not faced before. This panel examines whether that scrutiny has actually altered how these intermediaries structure rebate guarantees, formulary placement terms, GPO fees, and wholesaler chargeback arrangements, or whether the core economics have simply adapted around new disclosure requirements while leaving the underlying incentive structures intact. Panelists will discuss what manufacturers, health systems, and payers should expect going forward: are we seeing genuine renegotiation of the value chain, or a more sophisticated version of the same model built to withstand the next round of enforcement.

BEYOND MONITORING: USING LONGITUDINAL PATIENT DATA TO ANTICIPATE AND ORCHESTRATE CARE
Oncology monitoring today is fundamentally reactive. A patient reports a symptom. A nurse responds. Even the best-implemented ePRO platforms are built around this loop: capture the signal, escalate the alert, manage the episode. This session proposes a different question entirely: which patients three weeks from now is most likely to experience a treatment-limiting toxicity, an unplanned admission, or a dose reduction; what do I do about it today, before the deterioration event occurs? This shift from surveillance to foresight changes not just how clinicians respond to patients, but when and how they structure care decisions across an entire patient population.  This session is an honest account of where the science stands, what prospective validation requires, and an open invitation to the community oncology leaders in the room to become part of it.

OBESITY, GLP-1s AND CANCER PREVENTION: ARE WE WITNESSING THE NEXT REVOLUTION IN ONCOLOGY?
GLP-1 therapies are transforming obesity management, but their potential impact on cancer prevention may be even more significant. Emerging evidence suggests meaningful reductions in obesity-associated cancers, yet important scientific questions remain. Are observed benefits driven solely by weight loss, or do GLP-1 therapies exert independent biologic effects? As employers redesign benefits, Medicare launches new demonstrations, and manufacturers pursue additional evidence, oncology stakeholders must begin preparing for how obesity treatment could fundamentally alter cancer incidence, screening strategies, payer policy, and long-term healthcare spending.

WHO ACTUALLY PAYS WHEN THE ONCOLOGIST PRESCRIBES THE DRUG?
The fundamental tension in oncology value-based care remains unresolved: the clinician making a six-figure treatment decision often operates within a reimbursement model that directly benefits from higher drug spend. While the Enhancing Oncology Model (EOM) and other risk arrangements have created new accountability structures, they have not fully realigned incentives at the point of prescribing. As biosimilar adoption, site-of-care optimization, and dosing reform become increasingly important cost-management levers, the industry must confront the question of who truly bears financial responsibility for drug utilization. The discussion can examine whether current payment models are capable of delivering sustainable oncology affordability without changing prescribing incentives.

GERIATRIC ONCOLOGY IS THE NEXT MAJOR VALUE-BASED CARE OPPORTUNITY
Chronologic age is a poor predictor of treatment tolerance. Functional status, cognition, and polypharmacy often matter more. Yet these factors remain inconsistently incorporated into treatment decisions. As the oncology population ages, risk-bearing organizations have an opportunity to operationalize geriatric assessment at scale and develop clinical models aligned to the specific needs of this population.
This session will look at the measurable implications of unaddressed geriatric oncology needs, specifically on toxicity, hospitalization, treatment adherence, and total cost of care.

THE PRIVATIZATION OF MEDICARE, ADVANTAGE OR NOT?
As the debate over Medicare’s future intensifies, oncology leaders are grappling with the potential impacts of increased privatization on cancer care delivery. The push towards expanding Medicare Advantage (MA) plans, championed by some policymakers, promises benefits like care coordination and supplemental services. However, it also raises concerns about access, administrative burdens, and financial toxicity for cancer patients. Recent studies have highlighted disparities in cancer treatment access between traditional Medicare and MA plans, with MA enrollees facing more limited provider networks and potential delays in receiving high-cost drugs. Further, the implementation of prior authorization requirements by MA plans has led to treatment delays and increased physician burnout. While MA plans offer potential cost savings and additional benefits, they also introduce complexities in coverage decisions and may limit patients’ choices of specialists and treatment centers. This panel will discuss the pros and cons of Medicare privatization and its impact on the quality and accessibility of cancer care for America’s seniors.

DETECTING VALUE: THE FRAMEWORK FOR MEASURING CLINICAL UTILITY OF SCREENING PROGRAMS
Early detection of cancer has been the focus of major initiatives across societies, public outreach programs, payers, and regulators. However, the framework for assessing the effectiveness and clinical utility of primary-care screening programs is under-examined. Drawing on the procedures of the U.S. Preventive Services Task Force (USPSTF), this panel will examine the critical chain of evidence required to demonstrate that screening does more than detect disease– such as, is there an identifiable at-risk population? Does earlier detection change care or outcomes? How does early detection impact false positivity rates? Do intermediate or biomarker outcomes predict morbidity and mortality? What are the potential harms or unintended consequences of screening and treatment?
Experts in methods for detecting cancer, and in study methods required, will link these questions into an analytic framework and discuss the issues, complexities, and decision-making associated with screening programs via a case study on the topic of Colorectal Cancer. This case study will cover existing screening programs and technologies, the evolving evidence base on patient benefit, and the current reimbursement landscape. Panelists will discuss how payers, HTA bodies, clinicians, and policymakers use these frameworks and the role of real-world evidence, economic modeling, and longitudinal evidence synthesis to evaluate screening programs. Attendees will gain practical insights into a central question facing healthcare systems worldwide: how can we assess when cancer screening delivers sufficient clinical and economic value to warrant widespread adoption and reimbursement?

BI-SPECIFICS: COME OF AGE WITH A NO MARGIN, NO MISSION REALITY
To quote a legendary colleague in cancer care, this past Dec ’25 the Bi-Specifics outcomes data at ASH was amazing!  Financing is a problem in this sector because it is very expensive and clinical techniques for the “start-up” dosing of bispecifics (the first 6-8 weeks) are currently badly under-reimbursed by all payers. The oncology landscape of 2026 is dominated by therapies with binary outcomes: they either offer a potential cure or cause life-threatening systemic toxicity. For patients on CAR-T, Bi-specifics, or chronic TKI/IO combinations, there is no “off-ramp.” Discontinuing treatment due to unmanaged side effects isn’t just a clinical setback; it is often a terminal event. In this “no-fail” environment, the success of a half-million-dollar therapy depends entirely on the precision of monitoring between clinic visits. This session explores the operational and clinical shift required to manage these high-stakes therapies. We will discuss why traditional reactive triage is obsolete for drugs that can trigger cytokine release syndrome (CRS) or neurotoxicity in hours, and how AI-driven longitudinal monitoring is the only way to move these therapies safely into the community and outpatient settings. This panel will discuss the discourse between innovation and the last of proper access process and reimbursement as the reality facing physicians and patients today. The panel will explore the topic in hopes of making recommendations to payers on how to solve the mission access when the science has never been better.

AUTOMATED ADAPTIVE RADIATION THERAPY
Adaptive radiation therapy (ART) traditionally involved re-imaging and replanning a radiation treatment plan when a cancer patient’s anatomy changed during a course of treatment (either through weight loss, tumor shrinkage, or physiologic changes). This was high resource work that can now be completely automated and completed with a touch of a button with the patient still on the linear accelerator table. Should the value of automated ART be equivalent to manual ART? How often can ART be reasonably used during a radiation treatment course? Would automated ART exacerbate disparities in rural, less resourced areas?

FROM OCM TO EOM TO AI: CAN BETTER MEASUREMENT MAKE CANCER CARE VALUE-BASED?
For more than a decade, value-based cancer care has promised to reward better outcomes rather than higher volume. Yet cancer care still largely runs on familiar fee-for-service economics: expensive drugs, ASP-based payment, pathway debates, and quality measures that often lack clinical force. The Medicare Oncology Care Model showed the problem clearly: it produced modest reductions in episode payments, but no significant improvement in hospitalizations, emergency department visits, or quality, and it generated an estimated net loss to Medicare after enhanced services and performance payments. CMS then designed the Enhancing Oncology Model to be more patient-centered and data-forward, adding ePROs, health-related social-needs screening, equity plans, CEHRT, and CQI data use. This panel will ask whether the missing ingredient has been measurement itself. The central question is whether AI, EHR-derived measures, mCODE/USCDI+ Cancer, and digital quality measurement can move VBCC from claims-based cost control toward clinically meaningful improvement for patients. Learn what OCM actually taught us, why CMS EOM  may still not be enough, why measurement may be the “missing operating system” for VBCC and whether a new era of AI and digital oncology data can create a plausible V3 model and will AI-EHR and digital quality measures can make oncology performance measurable in ways that are clinically meaningful, auditable, and actionable for patients. VBCC has not failed because the aspiration was wrong; it has stalled because oncology has lacked a usable measurement system.  The question for 2026 is whether AI, ePROs, EHR-derived data, and oncology interoperability standards can finally make value measurable enough to change care for patients.

ORAL ONCOLOGY LAUNCH STRATEGY: DESIGNING ACCESS, DISTRIBUTION, AND ADOPTION IN COMMUNITY ONCOLOGY
The continued growth of oral oncology therapies is reshaping how community oncology practices evaluate, adopt, and operationalize treatment. Unlike infused therapies, oral agents place significant responsibility for prescribing, adherence support, financial navigation, and ongoing patient management into already resource-constrained practice settings. For emerging manufacturers, launch success increasingly depends on early alignment across contracting, distribution, manufacturing, and practice-level execution within the community oncology ecosystem.
This panel would explore how oral oncology therapies move through community oncology networks and where early commercialization decisions most often determine success or failure. Discussion topics would include contracting strategies and their impact on access and adoption; distribution design ranging from closed/OOLD models to open distribution networks; and the downstream effects these decisions have on practice workflow, patient access speed, and adherence support services.
The conversation would also address critical upstream considerations, including manufacturing readiness, packaging design for real-world use, hub services, patient and provider support resources, and the role of advisory boards in shaping community-aligned commercialization strategy.
Overall, the session would focus on when to engage stakeholders, who needs to be aligned early, and what infrastructure should be established prior to launch to support successful adoption in today’s value-driven oncology environment.

FROM MOLECULAR INSIGHT TO PATIENT IMPACT: CASE STUDIES FOR IMPROVING PATIENT OUTCOMES WHERE MOST CANCER CARE HAPPENS
Most people with cancer in the United States receive their care in community settings, where even small delays in diagnosis, testing, interpretation, or follow-up can shape the entire patient journey. This session will highlight two efforts aimed at making molecular medicine more usable in the places where most patients are seen.
Dr. Juvianee Estrada-Veras will discuss return of results through his ongoing experiences with Murtha Cancer Center’s longitudinal cohorts, with an emphasis on how patients, clinicians, and care teams can be supported when molecular findings move from research into clinical context. James Thackeray will describe Lumea’s work in urology, where digital pathology and workflow redesign are being used to improve the path from biopsy to diagnosis, molecular testing, and treatment decision-making. Dr. Nathanael Fillmore will bring a national VA perspective through real-world immune checkpoint inhibitor outcomes research, including what large health systems can teach us about treatment benefit, patient heterogeneity, and the gap between clinical trials and routine care. Dr. Alexander Arrow will then discuss an emerging approach to counteract tissue insufficiency in community hospital biopsies, to allow more patients to be sequenced and therefore have access to precision medicine.
Together, these examples will explore a practical question for value-based cancer care: how can molecular information become not only more advanced, but also more timely, understandable, and useful for the 85% of patients cared for in the community?

THE RADIATION “BLACK BOX”: WILL EPISODE-BASED PAYMENTS FINALLY ALIGN RAD-ONC WITH VALUE BASED CARE?
Radiation oncology remains one of the most siloed and fee-for-service-heavy sectors in cancer care. While the medical oncology side has matured through OCM/EOM, rad-onc is just now facing the reality of the ROCR Act and bundled payments. This panel will tackle the friction between high-cost technical innovation (Proton, SBRT) and the payer’s drive for evidence-based “shorter is better” fractionation. We will also dive into the “PA Paradox”—whether automated prior authorization is a tool for guideline adherence or a barrier to timely, curative care.
